FEES AND PAYMENTS

You pay me at the beginning of the session and I will give you a statement you can submit to your insurance company. I accept cash, check, VISA, MasterCard, or Discover. Sessions cost $245 and last 50 minutes. Please come 20-30 minutes before your first session to fill out paperwork that will be helpful for you.

INSURANCE

I do not bill insurance-- I will give you a statement containing necessary medical billing information that you can submit to your insurance program, so they will reimburse you directly.

DIRECTIONS TO MY OFFICE

My office is located at 11980 San Vicente Blvd, Suite 620, in the Brentwood area of Los Angeles. The two nearest cross streets are Bundy and Montana.

Coming from Los Angeles:

1. Take the 405 Freeway to Wilshire Blvd West exit.

2. Continue West on Wilshire and turn right onto South Barrington Ave.

3. Continue on Barrington for 2 blocks, then turn left on San Vicente Blvd.

4. Continue West on San Vicente Blvd several blocks to Dr Burgess' office, on the left at 11980 San Vicente Blvd, Room 620.

Coming from Santa Monica:

1. From PC 1 turn left on Ocean Avenue next to the beach in Santa Monica.

2. Drive North on Ocean Avenue until you reach San Vicente Blvd.

3. Turn right on San Vicente Blvd and continue East until you reach Dr Burgess' office, on the right at 11980 San Vicente Blvd, Room 620.

PARKING

There is valet parking inside the building, but I do not validate. There is limited parking at meters surrounding my office. Beware, Brentwood tickets freely.

NOTE

The city of Brentwood is located between the cities of Westwood and Santa Monica. Please note that there is another San Vicente Blvd in West Hollywood, but that is NOT the location of my office.
